The Final Fieldwork Verification Form is the one fieldwork document that goes to the BACB with your certification application. Your monthly forms stay in your files unless you're audited; the F-FVF is built from them and submitted. You complete one per supervision structure; your responsible supervisor signs it, and, unlike the monthly form, it has no signing deadline. Multiple F-FVFs can go into a single application when your hours came from several experiences.

What is the F-FVF and how is it different from the monthly form?

It's the cumulative form you submit, built from the monthlies you keep.

At the end of a fieldwork experience, you and your responsible supervisor use the signed monthly forms to total the hours that count, and record them on the F-FVF. Two differences matter. The monthly form must be signed by the last day of the calendar month following the supervision month, while the F-FVF can be signed at any time. And only the F-FVF is submitted with your application; the monthly forms are what an auditor asks for.

That last row is a real difference. On the monthly form, only the responsible supervisor needs to be listed and sign. On the F-FVF, everyone from the organization who supervised you must be listed, even though the responsible supervisor is still the only signature.

When does a fieldwork experience end?

When the supervision relationship ends, and also when you take a break.

The obvious triggers are finishing your hours or the contract concluding. The one that surprises people: per the Handbook, taking a break from accruing fieldwork under the same supervision contract constitutes a new supervision experience. Pause and restart with the same supervisor, and you have two experiences, which means two F-FVFs. This is why some candidates submit three or four final forms rather than one.

How do I record concentrated hours on the F-FVF?

Actual hours, never the multiplied total, and in a separate section.

Concentrated supervised fieldwork carries roughly 1.33 times the value of supervised fieldwork toward BCBA certification, and about 1.3 times toward BCaBA certification. That multiplier is for your own tracking to see whether you've met the requirement. On the forms, you record the hours you actually worked. The F-FVF documents supervised and concentrated fieldwork in separate sections, so the board applies the weighting itself.

The practical version: keep a personal running total with the multiplier applied so you know where you stand, and report raw hours on every form. If you're audited, you'll need to show which hours were concentrated.

What order do I do things in?

Total your monthlies, get each F-FVF signed, then apply.

  1. Confirm every month is documented. A lost monthly form means those hours cannot be counted, and if an audit finds them missing, you may need to complete additional fieldwork.
  2. Total the hours from your signed monthly forms, separating supervised from concentrated.
  3. Complete one F-FVF per supervision structure, listing every supervisor involved in that experience.
  4. Get the responsible supervisor's signature. They must have been qualified to serve in that role for the entire duration of the supervision they're certifying. Electronic signatures are acceptable under the BACB's signature policy.
  5. Save the PDF, then reopen the saved file and check it. The BACB warns that if the file saves incorrectly and data is missing, the submission won't be accepted. Fill these forms in Adobe Acrobat Reader on the desktop, not in a browser, or the dates won't save.
  6. Submit with your application, attaching multiple F-FVFs if your hours came from multiple experiences.

What goes wrong at this stage?

Outdated forms, missing supervisors, and files that didn't save.

Three failures account for most of the trouble. Using an outdated BACB form or a non-BACB form, which will not be accepted at all. Omitting a supervisor from the F-FVF's list when several people supervised you at one organization. And submitting a PDF whose fields silently empty on save, which is why the BACB tells you to reopen the saved file before sending it.
