Multiple supervisors verification form

When I go to submit for my verification form it only says individual supervisor. I have three and I’m wondering how can I make it multiple supervisors?

I am having this issue as well on here. I went back to look and see if it was a similar issue that Samuel stated in a previous comment, but each supervisor is showing for the sessions they supervised so this does not seem to be my issue. However, I am still only seeing the one option-the individual supervisor form. I am following this to see if anyone else has found a fix to this issue.
This happened to me too, I had to go back and edit the sessions to add the other supervisors names when they were the ones supervising. It automatically updated to multiple supervisor form.
