Distorted monthly verification form

Good afternoon! Every time that I try to scroll down to sign a supervisee’s verification, the verification form looks distorted and I am unable to view it. It does the same thing when I try to save as a PDF. I tried to paste a picture in for reference and was unable to. Has anyone else experienced this or is it specific to my device?
