If supervisor has given more than 5% of her supervision (Unrestricted Individual and Group) for that respective month, then in that case I can add all of those or we have to adjust Supervised hour to fulfil the 5% BACB Requirement?

Minimum 5% requirement is what needed and if you ahve more than 5% supervision, it is added bonus. Include this in the data entry.
